### What a first visit looks like

A full psychiatric evaluation runs about an hour. We go through what brought you in, your history, what you have tried before and how it went, physical health and medications, and what you want to be different. You leave with a working diagnosis where one is clear, a plan, and a follow-up booked — not a referral to somewhere else.

Follow-ups are shorter, usually every few weeks at first and further apart once things are steady. Medication changes are made deliberately, one at a time, so it stays possible to tell what is working.

## Your plan is very probably one we bill

Usually, yes. We are in network with 19 commercial carriers, including Aetna, Anthem, Beacon Health Options, Carelon, Cigna and others — you can check your own plan below before booking anything. That list includes TRICARE and TriWest, so service members, veterans and military families are billed like anyone else here.

**We do not currently take Medi-Cal managed care.** If that is your coverage, we are a self-pay option rather than a covered visit, and it is better to know that now than at the front desk. Self-pay is $250 for a 60-minute initial psychiatric evaluation and $150 for a 30-minute follow-up visit, and there is a sliding scale — please ask.

For most people reading this in Redondo Beach, the insurance question is already settled. Of working-age residents, 82.9 percent hold commercial coverage, employer plans plus those bought directly, and those are the plans we contract with. Only 6.2 percent are on Medi-Cal, which we do not currently bill, and 4.5 percent are uninsured. Compare that with Huntington Park, fifteen miles inland, where 36.1 percent are on Medi-Cal and close to a quarter have no coverage at all, and you can see why the same practice writes two very different pages. Here, money is rarely the thing standing between someone and a psychiatric appointment.

Which leaves time, and a particular kind of reluctance that shows up in comfortable places. When nothing external is obviously wrong, it gets harder rather than easier to justify asking for help. People arrive having managed something for four or five years on the grounds that they have a good job, a decent house and no excuse. That reasoning is not clinical, and it costs those years.

The drive is about half an hour and the three routes are within a minute of each other. From City Hall on Diamond Street we measured 13.9 miles and roughly 29 minutes by 190th Street onto the 405, 14.3 miles and about 28 minutes straight down the 405, and 14.4 miles and about 29 minutes by Del Amo Boulevard, sampled on a Tuesday at roughly a quarter to twelve. One honest detail from that sample, since we record what the tool actually said: the option Google marked as the best route, the 190th Street one, came in a minute slower than the plain 405 route listed beneath it. That is a single reading at a single hour and we are not making a rule out of it. It is just a reminder that the recommended line is a forecast, and that leaving ten minutes early beats picking the clever road.

Rail is the odd part. There is a Metro station called Redondo Beach Station, and it is not in Redondo Beach. We put its coordinates through the Census Bureau’s incorporated-places lookup rather than trusting the sign, and it came back Hawthorne, about three and a bit miles from City Hall. The next two along, Douglas and El Segundo, are both in El Segundo. So this city has no rail station inside its own limits, which matters less than it sounds because none of them come our way anyway. A rail trip here means east on the C Line to Willowbrook and Rosa Parks Station, south on the A Line, then Long Beach Transit’s Route 51 along the boulevard. For the South Bay that is a long afternoon, and the video visit exists precisely for this.

About 2,643 people here are veterans, 4.9 percent of civilian adults, against 2.6 percent in Hawthorne and 5.1 percent in Lakewood. That is unsurprising in a city that has spent decades next to the aerospace corridor. TRICARE and TriWest sit on the same contracted list as every commercial carrier, bill the same way, and need no separate referral path.

There is a small piece of local history we like, because it undercuts the postcard. At the southeast corner of Harbor Drive and Yacht Club Way is California Historical Landmark No. 373, Old Salt Lake, where in the 1850s two men named Johnson and Allanson built works to make salt by artificial as well as solar evaporation. By 1879 the operation was yielding 450 tons a year. Before the beach city was a beach city it was an industrial salt works, which is a useful thing to know about places and about people: the current version is rarely the only one, and the earlier one is usually still under it somewhere.

What we actually do is narrow and we would rather be clear about it than sell it broadly. We are a psychiatric practice. That means an evaluation that ends in a working diagnosis, medication chosen with care and then genuinely monitored, and appointments that change course when something is not working. We do not provide weekly therapy. If therapy is what your situation calls for, or the half of it we cannot supply, we will say so at the first visit rather than leave you on a plan with a gap in the middle.

The pattern that suits this city is straightforward. Come in once for the assessment, because an hour in the room is a better basis for a diagnosis than an hour on a screen. Then hold the reviews by secure video, from home or from the office between meetings. Twenty-eight miles of round trip to confirm that a dose is working is a poor trade for an afternoon, and the review itself is identical either way.

We checked the coordinates against the Census Bureau's city boundaries and the K Line stop with your city's name on it stands in Hawthorne, about three miles from City Hall. Redondo Beach has no rail station of its own, and from that platform it is still the K Line, then the C Line east, then the A Line south, then a bus.

Which route should I take?

All three came within a minute of each other in our Tuesday late-morning sample, so take whichever you know. Worth one honest note: the route the app labelled the best, by 190th Street, was a minute slower than the plain 405 option listed under it.
