Paying for care

## Who pays, across a county of nearly ten million

We are in network with 19 commercial carriers, including Aetna, Anthem, Beacon Health Options, Carelon, Cigna and others, and you can check your own plan below before you book. TRICARE and TriWest are on that list, so service members, veterans and military families are billed like anyone else here.

**We do not currently take Medi-Cal managed care.** If that is your coverage, we are a self-pay option rather than a covered visit, and it is better to know that now than at the front desk. Self-pay is $250 for a 60-minute initial psychiatric evaluation and $150 for a 30-minute follow-up visit, and there is a sliding scale. Please ask.

### Coverage across Los Angeles County

The care

## Mental health services, the psychiatric kind

This is a psychiatric practice. We diagnose, we manage medication, and we keep seeing you so the plan keeps fitting your life. We do not run therapy groups or weekly talk therapy.

- ### [Psychiatry Services](https://breakthroughmentalhealth.health/treatments/psychiatry/)

Comprehensive psychiatric care, including diagnosis, treatment planning, and ongoing management led by a board-certified provider.

- ### [Medication Management](https://breakthroughmentalhealth.health/treatments/medication-management/)

Careful selection, monitoring, and adjustment of psychiatric medications to maximize benefit and minimize side effects.

- ### [Psychiatric Evaluation](https://breakthroughmentalhealth.health/treatments/psychiatric-evaluation/)

A thorough first assessment to understand your history and symptoms and build an accurate diagnosis and plan.

### The first appointment

Plan on about an hour. We ask what brought you in, what has and has not helped before, your health and current medications, and what you would like to feel different. You leave with a working diagnosis where one is clear, a plan, and the next visit already on the calendar.

Follow-ups are shorter. They come every few weeks while a plan settles and spread out once it holds, and any medication change is made one step at a time so its effect stays readable.

### Office or video, same care

Come to the office, log on from home, or mix the two. The appointment length and the clinician are the same either way.

New patients see [Irine Achuamang](https://breakthroughmentalhealth.health/providers/irine-achuamang/), a psychiatric-mental health nurse practitioner. [Dr. Cho](https://breakthroughmentalhealth.health/providers/austina-cho/) provides collaborating physician oversight.

## One practice, eighty-eight cities

Los Angeles County is a patchwork. It holds 88 incorporated cities, and about a million people live in unincorporated communities that belong to none of them. Our practice sits at one point in that patchwork: a single office on Long Beach Boulevard, near the southern edge of the county.

From there we see people across the whole county. How we see you depends mostly on where you live. From Long Beach, Signal Hill, Lakewood or Compton, the office is an easy trip. From West Covina, Pomona or Malibu it is not, and video makes more sense. On a Monday at 7.40am, in the morning peak, Google put West Covina City Hall about 1 hour 5 minutes from our door, Pomona City Hall about 1 hour 18 minutes and Malibu City Hall about 1 hour 9 minutes. Those are drives from the San Gabriel Valley and the Malibu coast. From the Gateway cities and Harbor communities near the office, the same morning’s trips ran from about 14 to 37 minutes.

## Two kinds of mental health services

When people search for mental health services in Los Angeles County, they find two very different systems. The county’s Department of Mental Health runs the public system, organized into eight service areas, and its 24-hour help line is the front door. It serves Medi-Cal members as their local mental health plan, and its services reach well beyond psychiatry.

We are a private psychiatric practice. We do one thing carefully: psychiatric evaluation, diagnosis and medication management, billed to commercial insurance or paid privately. If you have Medi-Cal, know that we do not currently take Medi-Cal managed care; the county is the local mental health plan for Medi-Cal and coordinates with the Medi-Cal health plans, so its help line is a sensible place to start. If you have a commercial plan, we may be in network, and the coverage check on this page will tell you.

## Coverage changes from one corner of the county to another

Across the county as a whole, about three in five working-age adults carry commercial insurance. That average hides a wide spread. In the Census subdivision covering South Gate and East Los Angeles, fewer than two in five do; in the South Bay Cities subdivision, more than four in five do. Roughly one in nine working-age adults in the county has no coverage at all.

That is why our pages for the county’s regions each describe their own coverage picture. If you live in the South Bay, the Gateway Cities, the Harbor Area or the north county, start with that page.

## The county’s own map of mental health care

The Department of Mental Health divides the county into eight service areas, and it helps to know yours before you call the county. Service Area 1 is the Antelope Valley, 2 the San Fernando and Santa Clarita valleys, 3 the San Gabriel Valley, 4 Metro Los Angeles, 5 West Los Angeles, 6 South Los Angeles, 7 East Los Angeles and 8 the South Bay, which takes in Long Beach too. Our office sits in Service Area 8, and we see people from all eight.

About a million people live in unincorporated communities, from East Los Angeles to Altadena, where the county’s Board of Supervisors acts as the city council. They are as much a part of the county we serve as any of the 88 cities.

## If you need help today

Planned psychiatric care is not emergency care. For urgent help anywhere in the county, call the county help line at any hour, or call or text 988. If someone is in danger, call 911.

## Los Angeles County questions

### Are you a psychiatrist's office or a mental health clinic?

A psychiatric practice. We provide psychiatric evaluation, diagnosis and medication management. Counseling, therapy groups and case management are mental health services too, but they are not ours. If one of those fits you better, we will tell you.

### How is your practice different from the county's mental health services?

The county Department of Mental Health runs the public system and is the local mental health plan for Medi-Cal, and its help line is the way in. We are a private psychiatric practice that bills commercial insurance or self-pay. Both are real options; which one fits depends mostly on your coverage.

### Do I need to come to Long Beach?

Not unless you want to. Cities close to Long Beach are an easy drive, and the far ends of the county are better served by video. In our Monday 7.40am sample, West Covina City Hall was about 1 hour 5 minutes away and Malibu City Hall about 1 hour 9 minutes, both in morning-peak traffic.

### Is this page the same as your Los Angeles page?

No. Our Los Angeles page is about the City of Los Angeles. This one covers the whole county, all 88 cities and the unincorporated areas between them.

### Do you see people in unincorporated areas like East Los Angeles or Altadena?

Yes. About a million county residents live outside any city, and they are as welcome as anyone in the 88 cities. Check your coverage, then choose the office or video.

### How do I know whether to call you or the county?

Start with your coverage. With a commercial plan or self-pay, you can book with us directly. With Medi-Cal, remember that we do not currently take Medi-Cal managed care, so the county help line at (800) 854-7771, the main way into its public mental health services, is a sensible first call.

## From any of the 88 cities, one practice

Ask for a time online and we will confirm it by phone. Tell us whether you would like the office or video, and we will set it up that way.
