### What a first visit looks like

A full psychiatric evaluation runs about an hour. We go through what brought you in, your history, what you have tried before and how it went, physical health and medications, and what you want to be different. You leave with a working diagnosis where one is clear, a plan, and a follow-up booked — not a referral to somewhere else.

Follow-ups are shorter, usually every few weeks at first and further apart once things are steady. Medication changes are made deliberately, one at a time, so it stays possible to tell what is working.

## About half of Hawthorne is on a plan we bill

Usually, yes. We are in network with 19 commercial carriers, including Aetna, Anthem, Beacon Health Options, Carelon, Cigna and others — you can check your own plan below before booking anything. That list includes TRICARE and TriWest, so service members, veterans and military families are billed like anyone else here.

**We do not currently take Medi-Cal managed care.** If that is your coverage, we are a self-pay option rather than a covered visit, and it is better to know that now than at the front desk. Self-pay is $250 for a 60-minute initial psychiatric evaluation and $150 for a 30-minute follow-up visit, and there is a sliding scale — please ask.

Hawthorne has three Metro rail stations inside its city limits, which is more than most cities on the way here have, and not one of them will bring you to this office. Both C Line stops send you east to Willowbrook and Rosa Parks Station, where the A Line turns south, and even then the train leaves you needing Long Beach Transit’s Route 51 to finish the boulevard. Two trains and a bus for a trip a car does in about twenty-five minutes. We lead with that because a page about a psychiatric office should tell you how hard it is to reach before it tells you anything else.

One of those three stations is called Redondo Beach Station. It is on the K Line, and it stands in Hawthorne. We ran its coordinates through the Census Bureau’s city-boundary lookup rather than trusting the name, and the answer came back Hawthorne. The city it is named for has no rail station of its own at all. None of this changes your treatment, but it is a fair example of how we handle facts here: checked against a primary source, and written down even when the tidy version would read better.

The drive is the strange part. When we sampled it on a Tuesday at about a quarter to twelve, Google offered three routes from City Hall on West 126th Street and all three took twenty-five minutes. The 405 at 14.4 miles, El Segundo Boulevard and the 405 at 16.0, the 110 and the 405 at 16.8. Same time, two and a half miles apart. What that tells you is that distance is not deciding this trip, the 405 is, and we did not sample the morning or evening peaks, which will be worse. If you are choosing between the three on a map, choose the one you find least stressful to drive. The clock will not notice.

On coverage, Hawthorne sits close to the middle and the middle is worth spelling out. Just over half of working-age residents, 51.3 percent, hold commercial insurance, and those are the plans we contract with. Around 27.6 percent are on Medi-Cal or another means-tested program, which we do not currently bill. Another 14.9 percent, roughly one person in seven, has no coverage at all. So for about half of the people reading this the insurance conversation is short, and for a substantial minority it is a different conversation entirely: self-pay at published rates with a sliding scale, or the county line on (800) 854-7771, which answers at any hour and can arrange a referral. We would rather point you somewhere useful than let you assume we are an option when we are not.

What we do is narrower than the phrase mental health services suggests. We are a psychiatric practice, which means a proper evaluation that ends in a working diagnosis, medication chosen with some care and then actually monitored, and appointments that change course when something is not working. We are not a counselling service and we do not provide weekly therapy. Where therapy is the right answer, or the half of the answer we cannot supply, we say so at the first visit rather than keeping you on a plan with a hole in it.

There is one piece of Hawthorne that has nothing to do with any of this and is worth a line anyway. At 3701 West 119th Street there is California Historical Landmark No. 1041, the site of the childhood home of the Beach Boys, where over Labor Day weekend in 1961 three brothers and two friends recorded a song called Surfin. A garage in a residential street, an ordinary weekend, and then a thing that outlasted everyone involved. We mention it because the people who come to us from Hawthorne are mostly in the ordinary-weekend part of their own story, worn down by something they have carried quietly for years, and it is easy to forget that the unremarkable stretch is where most of the work gets done.

Practically, the twenty-five minutes only has to happen once. Book the assessment in the room if you can manage it, then let everything after that be video. What we will not do is pretend the choice is neutral. A first appointment carries more when we can see how someone sits, waits and answers a question they were not expecting, and none of that survives a laptop camera especially well. A medication review three weeks later carries none of it, and driving the 405 twice for fifteen minutes of dosage arithmetic is a bad trade. Split the difference that way and Hawthorne stops being far.

## Frequently asked questions

Hawthorne has three train stations. Can I take one to you?

Not directly, and this surprises people. All three run the wrong way for us. Both C Line stops send you east to Willowbrook and Rosa Parks Station, where you change to the A Line south, then finish on Long Beach Transit's Route 51 up the boulevard. Two trains and a bus. For most people in Hawthorne the car or a video visit is the honest choice.

Why is Redondo Beach Station in Hawthorne?

Because stations are named for where the line was heading, not for the ground they stand on. We checked the coordinates against the Census Bureau's city boundaries and the station sits inside Hawthorne. It is a small thing, but it is the sort of small thing that sends someone to the wrong place, and we would rather put it in writing.

Which road should I take?

Any of them. That is the unusual part. In our Tuesday late-morning sample the 405, the El Segundo Boulevard route and the 110 all came out at about 25 minutes, and the shortest happened to be the one the app picked. Mileage is not the variable here. Traffic on the 405 is, and we did not sample the peaks.

Is my plan one you actually bill?

Probably. About half of working-age Hawthorne residents hold commercial coverage, which is what we contract with. Roughly one in four is on Medi-Cal managed care, which we do not currently bill, and about one in seven has no coverage at all. If either of those is you, we are a self-pay option with a sliding scale rather than a covered visit.

I get anxious in waiting rooms. Does that rule this out?

No, and it is worth saying that the first visit is the longest one you will have with us. If being in a room with strangers is part of what you want help with, tell us when you book and we will keep the wait short. Social anxiety is a condition we treat, not an obstacle to treating you.

Can attention problems really start in adulthood?

They do not usually start there, but they often get noticed there. Adults come to us having coped through school and then hit a job or a family where the coping stopped working. The assessment looks at how far back the pattern goes and where it shows up now. Arriving late to the question does not make it the wrong question.
